Job Summary and Qualifications

The Medication History Pharmacy Technician specialist performs medication histories for new hospital admission. Under general supervision of the clinical pharmacist, provide complete and accurate information for each medication, e.g., drug name, dose, route, frequency, indication, and date/time last dose was taken. Utilizes available resources for medication history collection including but not limited to: patient/family interview, medication lists, outside facility medication administration records, outpatient pharmacy records, and physician office records. Verifies/updates patient allergy, adverse drug reaction, preferred pharmacy and primary care physician information in the electronic medical record. Documents medication histories completed in the electronic health record, sending to the pharmacist for cosignature in an accurate and timely manner. Documents medication histories in CPW/VigiLanz in an accurate and timely manner. Assists the medication reconciliation pharmacist by ensuring patients identified for discharge counseling and medication reconciliation have a pharmacy services completed medication history. Prepares education materials for the medication reconciliation pharmacist as necessary. Schedule may vary according to the needs of the facility/department and may include weekend, evening shift, holiday, on-call, and additional hours.

Performs medication histories

Interviews patients and family members for the purpose of compiling an accurate and complete list of current medications the patient was taking prior to admission to the hospital, to include all prescription, non-prescription and investigational medications.

Contacts the patient’s community pharmacy, physician’s office, or other healthcare facility for clarifications if needed.

Accurately updates medication history, allergies, adverse drug reaction, pharmacy and primary care provider information in the electronic health record.

Correctly and completely documents interventions made to the medication history in the electronic medical record prior to the end of each shift worked.

·

Discusses any questions or issues that arise during the medication history process with a clinical pharmacy specialist.

·

Appropriately communicates unresolved medication history issues to clinical pharmacy specialists.

·

Documents patient medication histories in the electronic health record and CPW/VigiLanz.

·

Ascertains need for language assistance and obtains interpreter as necessary.

·

Assists the clinical pharmacist performing education and/or discharge counseling through identification of patients that need medication histories completed by pharmacy services and preparation of counseling materials

Maintains competence required for current position.

Maintains pharmacy technician certification and Pharmacy Technician Registration.

Attends and participates in staff meeting regularly (full-time and PRN staff). Obtains prior approval when unable to attend.

Attends orientation, education and training programs. Reviews literature and other materials pertinent to the practice of pharmacy.

Completes competencies/ skills assessments by established deadlines.

Completes competencies associated with Medication History Technician position.

Stays current with pharmacy communications.

Shares good catches with team and local leadership.

What qualifications you will need:

  • High school graduate or GED

  • 2 years previous retail pharmacy experience

  • Maintains current pharmacy technician registration or pharmacy intern license
