JOB TITLE: Case Manager Transition Program

LOCATION: Murrow Indian Childrens Home Muskogee, OK

SUPERVISOR: Executive Director

EMPLOYEE STATUS: Full-time, Exempt

EMPLOYEES SUPERVISED: None

POSITION SUMMARY

Under the direct supervision of the Executive Director, and in keeping with the Mission Statement, the Murrow Indian Childrens Home policies and procedures, and administrative accountability, the Case Manager will work closely with and guide the students to ensure success in the program by maintaining daily contact and routinely assisting clients with daily needs. The Case Manager will provide educational opportunities in independent living skills and maintain aftercare plans for clients leaving care. The Case Manager will foster moral, spiritual and cultural development of the clients. This program helps nurture lifelong connections, empowers youth to continue their education, helps them develop good coping skills, and decision-making skills. You will meet with the therapists to help build treatment and educational plans. Be aware and open to alternative education styles. Implement assessment tools and develop individualized transition plans. Submit a copy of all plans to the executive director and for those youth that are in tribal custody, submit medical, educational, etc. reports to the program manager.

The Case Manager will handle routine administrative functions and maintain confidentiality of clients and program information. Participate in and volunteer for all Murrow fundraisers.

Prior to employment, complete and pass a finger printed Criminal Background Check, Child Care Restricted Registry Check, and thorough reference verification.

Submit to and pass a Physical and Drug Screen.

INDEPENDENT & TRANSITION LIVING

  1. SPECIFIC DUTIES AND PRIMARY RESPONSIBILITIES
  2. Recruitment Develop a plan for recruiting youth
  3. Attend ICW meetings and meet with case workers to recruit youth for the program
  4. Assist in completion of forms, the Casey Life Skills Assessment and any other skills assessment.
  5. Develop a realistic individualized plan for each young adult and revise as needed. Submit a copy to executive director.
  6. Attend meetings with executive director as needed.
  7. Organize and maintain good up to date client files.
  8. Serve as a role model for students by exhibiting skills expected from students
  9. Complete requests for childrens Independent Living funds, OKSA and tribal funds and services.
  10. Follow the Seven Key Elements to Transition and help young adults develop the skills with-in each element.
  11. Assist them with securing employment.
  12. Attend Church with the young adults.
  13. Perform initial life skills assessment and maintain quarterly assessment on all students in care

EDUCATIONAL

Attend IEP Meetings on Transitional young adults that are eighteen and over, but still attending high school

Monitor progress of young adults in the program

Coordinate with school personnel in order to provide maximum probabil
