Job Summary:

Under the supervision of the Population Health Care Director, the CHW/Promotra/o will support population health efforts by providing telephonic and in person support to patients by identifying barriers to accessing health care, providing social support and health education. The CHW/Promotora/o will serve as a liaison/intermediary between individuals, communities and health and social services to facilitate access to care, improve the quality and cultural responsiveness of service delivery, and address social determinants of health. Works in both clinical and community-based settings, including client's homes.

Core Competencies:

 

  • Experience working with Medicare, Medicaid or Special Needs populations.

  • Ability to connect with people and understand the challenges they face.

  • Ability to use a range of outreach methods to engage individuals and groups in diverse settings.

  • Well connected to the community and resources within the community they will serve.

  • Effective written and verbal communication skills demonstrating respect and cultural awareness during interactions with patients.

  • Ability to travel throughout the assigned region and comfort with conducting home visits.

  • Understanding of the healthcare system and clinic operations

  • Fluent written and verbal skills in English and Spanish preferred

    Job Requirements:

  • High school plus specialized, short-term training in clinical, medical skills

  • Certified Community Health Worker required

  • 3+ years of experience in a health care setting

  • 1+ years of experience with Microsoft Suite Outlook, Word, PowerPoint, and Excel

    Proficient computer skills to work efficiently with electronic medical records
