About the Role: Care Access Navigator
Our company is growing, and we believe that our team is the key to our success! We are looking for a motivated, organized, and people first-oriented Care Navigator to ensure timely submission of medical documentation and insurance authorization requests.
Care Navigator Essential Functions:
Serve as a brand ambassador for Vitra reflecting our vision, mission, and values
Show a genuine interest and compassion for the communities we serve and commitment to the diversity of our clients and team members
Ensures timely submission of medical documentation and authorizations requests
Assists with administrative tasks including answering phones, directing calls, email correspondence, photocopying, sending faxes, scanning, filing, etc.
Escalates client concerns and/or progress concerns to management as necessary
Works with the Care Access Manager to ensure incoming and existing clients have updated medical docs in charts
Maintains confidentiality of records in compliance with applicable laws and regulations
Review incoming intake paperwork, prepare all required forms, and send initial requests for medical documentation
Maintain database of Primary Care Provider contact information and confirm eligibility as MassHealth Providers
Complete a high level of calls to doctor's office and hospitals to ensure timely return of outstanding documents
Employ outstanding time management skills to ensure all new cases have forms sent out in a timely fashion while performing regular follow up on all outstanding requests
Prioritization of workload based on clients' length of time in and progress through the intake process
Performs any other duties as assigned
Care Navigator Requirements:
Bachelors degree preferred
Fluency in English and Spanish highly preferred
Previous experience in home health care or other branch of health care preferred
Excellent interpersonal and communication skills required
2+ years prior experience with high volume record processing required
Creative problem-solving skills
Demonstrated success with data-driven outcomes
Customer service mentality
Excellent time management skills
